Directions to Agency

Middletown Offices/Main Campus

From Route 17 West Bound:
Exit 122 at Crystal Run Road from Route 17.
Turn Right at stop light. Go to left hand lane to prepare for left hand turn..
Turn left at stop light on to Crystal Run Road.
Travel to the very end of this road and it will “T” at Goshen Turnpike.
Turn right at stop sign on to Goshen Turnpike.
Make next left on to Stony Ford Road.
Travel 0.6 miles. Just after hard right turn travel by a grey ranch house.
See Crystal Run Village sign.
Make right into driveway after 3-story beige Victorian house. Park in lot.
This is the reception building.
Go to front entrance and you will be directed to appropriate building.
 
Directions From Route 17 East Bound:
Exit 122 at Crystal Run Road from Route 17.
Turn right at stop light on to Crystal Run Road.

 Sullivan County Office in Monticello

From East or West Bound Route 17
Take Route 17 Westbound or Eastbound to Exit 105A/NY 42 South.
Exit Ramp feeds onto Pleasant Street/Route 42 South.
After seeing Monticello Police Station, at the 3^rd traffic light, turn Right on to West Broadway/Route 42 South.
Continue to Landfield Avenue, make Right turn.
(if you see stone church or Post Office you have gone past the turn)
Parking lot is on the left hand side before the second building.
Crystal Run Village, Inc. office is in second building on the left.

 Rockland County Office in Tallman 

From South or North Bound I-87/NYS Thruway
Travel Route I-87/NYS Thruway to Exit 14B/Airmont Road.
If traveling South bound, Exit 14B and turn Right on to Airmont Road, if traveling North bound, Exit 14B and turn Left on to Airmont Road.
Move to Left hand lane after crossing Railroad Tracks.
Turn Left on to Route 59. Travel 0.8 miles.
Arrive at #321 Route 59, Hillmen Plaza (across from Wendy's) Turn Right to enter Hillman Plaza. Stay right, travel up the hill to the rear parking lot. See office sign and entrance.
