Residential Opportunities

Group Residences
The Crystal Run Village, Inc. reconfiguration plan initiated in 1991 with the relocating of people housed at Greerwoody-Crest, a 90 bed Intermediate Care Facility in Pomona. The agency’s Fallsburg campus and original residential school in Middletown are projected to close in early 2002. The organization’s rigorous development campaign will have succeeded in providing the residents of these large congregate care facilities with the opportunity to move into supervised group homes throughout Orange, Rockland and Sullivan Counties. Forty-three houses in total, accommodating 4-14 people, will offer individualized services, providing a path to normalized living and community participation.
The Crystal Run Village, Inc. Residential Services Program offers optimum community inclusion, comfort, enjoyment, convenience, beauty and security. Suburban and rural environments offer easy access to the outdoors, shopping, amusements, neighborhood happenings and religious services. The services provided in these home-like settings include 24-hour staffing, meals, daily program planning, transportation, medical assistance and help in defining personal preferences and individual goal achievement.

Supportive Apartments
This independent living program services people who are fully integrated in the community and are designed to serve people with mild or moderate mental disabilities who do not need 24-hour supervision. The supportive apartment program provides a case worker as auxiliary support for the continued success of their independence.

Respite 
In addition to permanent housing, the agency also provides temporary housing through its Respite House in Orange County. The service is designed to provide family caregivers with short-term relief from the demands of caring for their child (12 years or older) or adult relative with a disability.

Nursing Supervision
Crystal Run Village, Inc. provides a Registered Nurse/resident ratio that ensures the individualized quality of care and safety of participants in its residential programs. The agency’s skilled nursing department is comprised of 13 Registered Nurses and 8 Licensed Practical Nurses who are responsible professionals and devoted care givers. Residents requiring immediate nursing care, those with chronic need or individuals experiencing a common illness, benefit from a well-planned triage nursing system in collaboration with residential staff.
The nursing department provides unlicensed direct care staff with initial and on-going training in tasks commonly identified as nursing procedures, such as, medication administration, diabetes management or medical equipment needs. High standards and continued supervision of in-home health services support the quality of life in Crystal Run Village residences.
The nurses observe, assess, plan, implement, and evaluate. They provide w
eekly over-site to their assigned residences to maintain compliance with state mandated medical standards. And, they work closely with the community based healthcare providers of the people served by the agency to achieve the best care possible.

Service Coordination
The Crystal Run Village, Inc. Service Coordination Department provides services to people living within the agency’s residences, as well as, to individuals that may be living at home with their families, in residences operated by the state or by other agencies, or on their own in the community.

People eligible for services must be at least three years old with a developmental disability, and meet stipulated levels of care requirements. The service provides not only a system of supports, but benefits the people with a disability and their families by allowing for the opportunity to make informed choices, resulting in a flexible, personalized plan of care that adjusts with an individual’s age and life changes.
The program’s goals emphasize: self-determination and choice; empowerment and freedom; integration; and productivity through meaningful contributions to society.

Mental Health Programs
Independent Living
Supported and/or Supervised Housing is available through the Crystal Run Village, Inc. Mental Health Programs for
individuals and families with the goal of independent successful living. The Crystal Run Village, Inc. Supported Housing Services Program provides independent living opportunities and case management services to people with mental illness or those dually diagnosed with a developmental disability living in Orange County. The program fosters an atmosphere of security and consistency of quality of life with the objective of self-sufficiency for the individuals and their families.  
Case Managers negotiate and secure community resources, which may include: housing; employment; financial assistance; 24-hour crisis intervention; children’s and family services. Enhanced assistance can be provided in the home and in the community in areas such as: life skills training;  job coaching; and mentoring. The Crystal Run Village, Inc. Mental Health Program provides services to approximately 107 people in their communities.

Invisible Children’s Program
The Invisible Children's Project is a nationally recognized program that originated in Orange County. The focus of the program is to give parents with a mental illness the tools for providing a nurturing and safe environment for their children.  Support services can include: family case management; respite child care; parent support groups; in-home clinical consulting; parenting education; financial aid access; family recreational activities; children’s art therapy; vocational and educational services; medical/medication counseling, and referrals.

Orange County Single Point of Entry
A new initiative of the New York State Office of Mental Health (OMH), Single Point of Entry is intended to provide adults with mental illness a simplified process for accessing community support services. The Care Coordinator functions as a liaison between the Orange County Department of Mental Health (OCDMH) and community-based residential and case management service providers. The service offers a reduction in the time from referral to receipt of services; increase in the number of high needs individuals receiving priority services; better community integration and community tenure, and realize improvements in service appropriateness and outcomes.

Vocational and Skill Development Services
Day Habilitation
The philosophy of making a difference by nurturing the abilities of each person is the operative focus of the agency’s Open Vistas Day Habilitation Program.  A high staff to participant ratio provides for an accomplishment-driven environment offering great potential for individual growth.
Field trips, cooking classes, sharing through community outreach, literacy/numeric training, fine motor skills development, sports and expression through art and performance are some of the programs designed to encourage the development of the individual’s socialization skills, confidence development and the understanding of opportunities through choice.

Vocational Services and Supported Employment 
Members of the Crystal Run Village, Inc. Vocational Services Team, work one-on-one with potential employment candidates with a disability, to ascertain job interests and performance abilities in order to provide optimum choice in locating an integrated workplace opportunity. Job coaching is provided through the job application and interviewing process.
Once in the workplace, on-going support provided in the form of training and advocacy ensures the individual’s success in performing job functions and congenial social interaction with co-workers and business management. Opportunities for employment are the result of strong cooperative relationships developed within the business community by the Crystal Run Village, Inc. Vocational Services Team.

Mobile Work Crew
Applying the principles of Supported Employment, Village Care is an independent affirmative business, providing landscaping and janitorial services to homeowners and businesses.
Village Care employs an integrated staff of people from the community and people with developmental and psychiatric disabilities. This mobile work crew model offers a competent, dedicated and friendly workforce in the competitive maintenance and lawn care marketplace, with all employees working side-by-side.
