Service Coordination

The Crystal Run Village, Inc. Service Coordination Department provides services to people living within the agency’s residences, as well as, to individuals that may be living at home with their families, in residences operated by the state or by other agencies, or on their own in the community.

People eligible for services must be at least three years old with a developmental disability, and meet stipulated levels of care requirements. The service provides not only a system of supports, but benefits the people with a disability and their families by allowing for the opportunity to make informed choices, resulting in a flexible, personalized plan of care that adjusts with an individual’s age and life changes.

The program’s goals emphasize: self-determination and choice; empowerment and freedom; integration; and productivity through meaningful contributions to society.

Medicaid Service Coordination

Families looking to better understand their child’s disability and the possibilities for their child’s future can get the guidance and services they need through Medicaid Service Coordination (MSC). MSC is a Medicaid State Plan service administered by the NYS Office of Mental Retardation and Developmental Disabilities (OMRDD) and is available to people of all ages.

MSC offers case management services to coordinate, plan and broker services that are often difficult to access due to fragmented service delivery systems. MSC activities are carried out by an identified and trained service coordinator to assist an individual in gaining access to desired services and needed supports.

Crystal Run Village, Inc. has come to understand that it is often the relationship that a person and their family develop with a service coordinator that influences the quality of the resources a family receives and the usefulness of the supports they are provided with. Crystal Run Village, Inc. provides the opportunity to develop the service coordinator partnership, essential for creating and maintaining a personalized plan designed specifically to the individual.

Service Coordinators work to:

  • Access community services
  • Make current services stronger
  • Avoid duplication of services
  • Develop resources when needed
  • Coordinate multiple providers ensuing services are provided as needed
  • Educate families on what they can expect from state and community agencies

MSC is a flexible system designed to grow and develop with the individual. People receiving MSC have the right to: elect the MSC provider of their choice; change their MSC service provider or service coordinator at anytime, and direct changes to individualized service plans.

Medicaid Service Coordination Eligibility

In order to receive MSC, a person must meet all of the criteria listed below. A person cannot be enrolled in any other form of comprehensive Medicaid long-term care service coordination program.

  • A person must have a documented diagnosis of a developmental disability
  • Be at least three years of age
  • Be enrolled in Medicaid
  • A person must demonstrate a need for ongoing and comprehensive, rather than incidental, service coordination
  • A person, or an individual authorized to give consent on behalf of the person, must choose to receive MSC
  • A person must not permanently live in an Intermediate Care Facility, Developmental Center, Psychiatric Hospital, Small Residential Unit, Hospital or Nursing Home, or any other Medicaid funded setting that provides service coordination.

Home and Community Based Services Waiver (HCBS)

HCBS provides a flexible funding system through the use of Medicaid dollars allowing a child with a disability to receive services through an individualized services approach. The waiver eliminates consideration of parental income for Medicaid eligibility for persons under the age of 18 who live with their parents. Children must meet eligibility according to the current OMRDD definition of developmental disability. It provides funding for services which in the absence of the waiver would only be available in an Intermediate Care Facility.

In-Home Residential Habilitation

In-Home Residential Habilitation is a Medicaid waiver program that addresses the needs of individuals with developmental disabilities living at home with their families. A trained staff person works one-on-one, an average of 10-hours a week, with the individual to increase skills and facilitate their independence, productivity, and integration into the community.

In-Home Respite Service

In-Home Respite Service is a Medicaid waiver program offering parents and care-givers short-term relief from their responsibilities. A qualified and trained staff person spends time with an individual engaged in a variety of activities meaningful to the individual.
